Medication Errors: The First Hour
A calm, ordered response to a medication error protects the person first and your service second. Here is the sequence.

Immediate actions
- Check the person. Observe for effects and seek clinical advice — 111, the GP, or 999 if they are unwell.
- Do not attempt to correct the error without clinical advice.
- Preserve the MAR chart and packaging.
Within the first hour
- Inform the person and, where appropriate, their representative — this is your duty of candour.
- Complete the incident record contemporaneously and factually.
- Notify the registered manager.
Notifications
Consider whether the incident is notifiable to the regulator, safeguarding, or the coroner. Record the reasoning either way.
Learning
A root cause analysis that stops at "staff error" has failed. Look at workload, interruptions, storage, labelling and competence.
